Registered Nurse Case Managers (RNCMs)

RNCMs are in charge of the patient’s care. They coordinate the patients’ care and are the bridge between the hospice care team and the patient and their families.

Licensed Practical Nurses (LPNs)

LPNs are nurses who assist RNCMs in caring for patients. They record what they observe and report their findings to the RNCMs.

Home Hospice Aides (HHAs)

HHAs are the aides who come and assist patients with ADLs, such as bathing and getting dressed.

Social Workers

Social workers assist patients and their family with concerns regarding end of life plans, power of attorney/living will documents, and other social needs.

Spiritual Counselor (Chaplain)

Spiritual counselors assist patients with their spiritual or religious needs through prayer or visitation.

Volunteers (Coming Soon)

Volunteers are individuals in the community who want to give back by spending time with patients and their families. These individuals can complete activities with patients or provide companionship.
